septic tanks, drainfield, gas and lawn sprinklers |

Provide
color samples of proposed paint color at time of permit submission. |

Garage
Sale Permit |
Miami
Shores Village requires a permit for a garage sale. One permit
allowed per year. |

Survey
Affidavit |
The
purpose of this form is to induce Miami Shores Village to issue a
building permit for the property without first providing a survey
less than 6 months old. |

Covenant
of Construction within Right
of Way

This form
shall be signed by the Homeowner when obtaining a permit for a
driveway in the Right of Way |

of Re-Occupancy |
This
permit is for the re-occupancy inspection, performed before the sale
of the house. |
| Declaration
of Use |
This form
is requested by the Planning and Zoning Dept, on special cases. |

of Commencement |
In order to be advised about
liens by contractors or materials suppliers who are not paid |

Contractor Registration Form |
If a
contractor wishes to pre-register with Miami Shores Village, this
form in addition to all required paperwork |

Owner Builder
Affidavit |
Owner of
property that is doing the work himself/herself, shall sign this
form in addition to the permit application |

Roof
Affidavit of Compliance |
This form
is signed by the Contractor after the Hurricane Mitigation work is
completed in compliance with FS 553.844 |

Roof Exemption
Form |
This form
is signed by the Homeowner when the actual value of the house is
less than $300,000, and submitted with a re-roofing permit by the
Roofing Contractor in compliance with FS 553.844 |

Roof
Inspection Affidavit |
This form
is signed by the Roofing Contractor and submitted to the building
inspector at the time of the tin cap inspection on a re-roofing job
in compliance with FS 553.844 |
